General Information, Insurance Policies & Emergencies
Cancellation Policy: Because of our commitment to providing the best possible experience to our patients, we require 24 hours advance notice to reschedule or cancel appointments; as such there is a $200 per hour fee for missed appointments or appointments rescheduled or canceled within the 24 hour period.
Dental Insurance Policy: Our office does not participate with any dental insurance, therefore payment is due at the time of your service. You may still be covered by your dental insurance plan when you visit our office if you are permitted to see an "Out of Network" provider. Check out your plan specifics or call your insurance carrier for benefit information.
Insurance Claim Forms: As a courtesy, we will submit your insurance claims electronically at then end of your appointment. Upon request, a copy of the forms can be generated for follow up purposes or so it can be submitted directly to the carrier.
Payment Policy: We currently accept Visa, Mastercard, American Express, checks and cash. Payment in full is due to Jonathan B. Levine at the time of service. Insurance claim forms are then submitted to their carrier for payment. Payment is issued and sent directly to the patient (subscriber). This should take around 2-4 weeks from date of service.
